At Triangle Care Management, we are dedicated to ensuring that LGBTQ+ individuals who are aging, or living with disabilities, receive the compassionate, affirming care they deserve. Our mission is simple: to create a personalized path to care that prioritizes comfort, safety, and dignity—without financial barriers.

Our care management navigation services are always free of charge, with donation options available for those who wish to support our mission.

Services & Programs We Provide
At the center of all Triangle Care Management services is a dedicated, one-on-one Care Manager Navigator (CMN) who both assesses your needs in a welcoming, affirming environment and educates you on the full-range of assistance available through both internal resources and external referrals.
Care begins at home. Non-medical care is not covered by medical insurance and is primarily paid for out of pocket. TCM will help you find or create the right setting for you. Sometimes receiving the care you need requires sharing your home or sharing a home with others.
The Embracing Aging Program (EAP) helps LGBTQ+ people create a community to improve their quality of life, to expand choices at all stages of aging, to sustain independence, and to remain in charge of their lives as they age. EAP communities are people who look out for each other and have a personalized path to care when they need it.
The Golden Girls sitcom made this option popular and it’s trending as a model for affordability and companionship. Residents share costs, including a caregiver. Housemates can be of similar age or of different generations. A shared housing residence will be managed by Triangle Care Management.
LGBTQ+ Affirming Care When You Need It.
To that end, we actively outreach to communities and individuals that represent diversity. Ideally, no LGBTQ+ person is left without care management navigation, regardless of race, ethnicity, gender, religion, sexual orientation, or socioeconomic status. Care management navigation is offered free of charge to everyone.

“If only I had a number to call…”
“I’m almost 80 years old living alone. I’ve been sick for several days and self-treating. It’s 2:30 a.m. and I’m having a prolonged coughing spell. Phlegm lodges in my throat; I can’t breathe. I grab my phone, unlock the door in case I have to call 911, and stop by the kitchen sink for a moment to calm myself. I soon begin to breathe. Later that morning, a friend calls just to say hi; I ask him to drive me to the ER. I sure wish my Mom was still around. I need someone to look in on me.”

The LGBTQ+ community has suffered enough. We still have many challenges, particularly as we age. It’s all about aging well! We need to look out for our seniors…we need to look out for each other. From checking in on each other every day, to receiving the care we need during a health issue, let’s leave no one without care.
